कृपया मुझे जन सूचना अधिकार– 2005 के तहत देश में चल रहे ‘नेशनल प्रोग्राम फॉर प्रिवेंशन एंड कंट्रोल ऑफ फ्लोरोसिस’ के बारे में निम्नलिखित जानकारियाँ देने की कृपा करें!
1- ‘नेशनल प्रोग्राम फॉर प्रिवेंशन एंड कंट्रोल ऑफ फ्लोरोसिस’ कार्यक्रम को उत्तर प्रदेश राज्य का कौन सा विभाग या मिशन देख रहा है!
2- उत्तर प्रदेश में ‘नेशनल प्रोग्राम फॉर प्रिवेंशन एंड कंट्रोल ऑफ फ्लोरोसिस’ को जो भी विभाग देख रहा है उसका पूरा पता तथा सम्पर्क दूरभाष दें!
3- उत्तर प्रदेश राज्य के कितने जिलों में यह कार्यक्रम चलाया जा रहा है!
4- उत्तर प्रदेश राज्य में ‘नेशनल प्रोग्राम फॉर प्रिवेंशन एंड कंट्रोल ऑफ फ्लोरोसिस’ का स्टेट नोडल अफसर कौन है तथा उनका सीयूजी मोब. नम्बर क्या है!
